The oldest historical nucleus, on the other hand, remains Montecatini Alto, a medieval village that preserves the urban layout, walls, and secluded atmosphere of past centuries, clearly distinct from the spa town that developed in the valley below.

The territory of Montecatini Terme combines the manicured greenery of the city's thermal parks with a typically Tuscan agricultural landscape, made up of hills cultivated with vineyards and olive groves surrounding the Valdinievole. Nearby lies the Padule di Fucecchio, one of the largest inland wetlands in Italy, an important refuge for migratory and aquatic birds, appreciated by those who love birdwatching and nature walks. Climbing towards Montecatini Alto, the view extends over the plain below and the first foothills of the Pistoia Apennines, offering a striking contrast between the urbanized valley floor and the still sparsely populated heights.
